Contact me for more information and fees at 914 450-4642 or sschulman@rcn.com
Payment is due at time of service, unless otherwise discussed. I accept cash or checks. Payment for your sessions includes communication with other members of your health care team; however, extensive phone conversations or emails will be billed. Please contact me to discuss my fees.
I do not accept insurance, however reimbursement may be available to you from your insurance carrier, depending on the type of plan you have.
Always call your individual carrier to find out your plan's specific benefits.
Many insurance companies do not cover medical nutrition therapy, or only cover specific diagnosis codes. Since I am not an insurance provider, you will need to have out-of-network benefits coverage to be eligible for reimbursement once you have met your deductible. It is best to speak to your insurance provider for more information about the services they will cover. Important questions to ask are how many sessions per calendar year will they cover and is there a pre-certification/pre-approval/pre-authorization process that needs to be completed prior to starting nutrition therapy with an out-of-network provider. I can provide you with a statement to submit to your insurance provider for reimbursement. You also may want to check with your employer as many offer flexible spending programs where medical nutrition therapy is an eligible expense.
The CPT code for medical nutrition therapy is 97802 for an initial evaluation, 97803 for follow-up sessions, and 97804 for group sessions.
Please feel free to contact me with any additional questions.
